Effective medication management is critical in family medicine to ensure optimal treatment outcomes and patient safety. This study examines the collaborative role of nursing and pharmacy professionals in improving medication management for family medicine patients. Nurses contribute through patient education, adherence monitoring, and individualized care, while pharmacists enhance medication safety by optimizing prescriptions and identifying potential drug interactions. The collaboration between these professionals fosters a patient-centered approach that addresses both clinical and psychosocial needs, reducing medication errors and improving overall healthcare quality. Evidence highlights the positive impact of interprofessional teamwork on treatment adherence, patient satisfaction, and clinical outcomes. To fully harness the benefits of this collaboration, healthcare systems should implement training programs and integrated care models that promote effective communication and shared decision-making among nurses and pharmacists. Strengthening this partnership is essential for advancing family medicine and achieving sustainable, high-quality care for patients.
