Abstract

Chronic diseases, such as diabetes, heart disease, and hypertension, pose significant challenges to healthcare systems worldwide. Nurses, as frontline healthcare providers, play a critical role in managing these conditions through continuous care, patient education, and interprofessional collaboration. In this review, we explore how nurses play an active role in chronic disease management, focusing on developing comprehensive care plans, continuous patient monitoring, and integrating digital technologies to enhance care delivery. The importance of communication, teamwork, and critical thinking in nursing practice is also discussed, along with the role of modern healthcare models in improving outcomes for patients with chronic diseases, particularly in Saudi Arabia. By emphasizing the multifaceted role of nurses, this review aims to underscore their pivotal contributions to improving the quality of life for patients with chronic diseases.
